The CCP Cares member portal offers Community Care Plan members a streamlined and secure online experience to manage their health benefits conveniently. Accessible at any time and from anywhere, this self-service platform provides personalized insights into health plan services and information in both English and Spanish.

Upon logging in with a confidential username and password, members can:

• View their or their child's virtual member ID card
• Check alerts and reminders
• Access details regarding coverage and benefits
• Review authorization and referral status
• Examine the explanation of benefits for themselves or their child

Additionally, members can search for:

• Doctors and healthcare providers
• Estimated costs for various services
• A wealth of health information through a comprehensive health library

The portal also facilitates several tasks, including:

• Changing the Primary Care Physician for themselves or their child
• Completing questionnaires and surveys, such as the Health Risk Assessment (HRA)
• Resetting their password
• Uploading health and fitness data, including information from the Apple Health app, when enrolled in self-tracking programs

Members have the option to personalize their experience by enabling Touch ID for login, selecting a preferred profile nickname, and customizing their menu to highlight favorites and shortcuts.
